HFRC provides individualised treatment programs for the management of musculoskeletal and chronic diseases by our expert team of Exercise Physiologists, Physiotherapists, Remedial massage therapists and rehab-in-the-home Nurses. We accept all referral types including GP Management Plans/Team Car Arrangements, Workers Compensation, Motor Vehicle DVA and NDIS. We have provided links below to commonly used referral types.

​Enhanced Primary Care (EPC) - GP Management Plan Referral (Medicare) - Team care arrangement referral for Chronic and complex conditions where a medicare rebate applies for Exercise Physiology or Physiotherapy.
​Group Diabetes Exercise Programs - Team Care Plan Referral (Medicare) - Team care arrangement referral for patients with Diabetes accessing group Exercise Physiology sessions.
Medicare rebate applies for Initial assessment and 8 group exercise sessions.
​DVA Referral - A D904 referral form is required for white and gold card holders, up to 12 sessions of Exercise Physiology or Physiotherapy prior to commencing treatment.
